What sildenafil is
Sildenafil is a prescription medicine for erectile dysfunction. It was approved in 1998 and it is the active ingredient in Viagra. Doctors have prescribed it for more than twenty-five years, so what it does, who it suits and what it can cause are all well known.

How it works, in plain terms
When you are turned on, your body releases a chemical messenger that tells the arteries in the penis to relax and open up. More blood flows in, the tissue swells, and that pressure squeezes shut the veins that would normally drain it away. That is an erection.
Your body also makes an enzyme called PDE5 that clears that messenger away again. That is why an erection ends. Sildenafil blocks PDE5. The messenger sticks around longer, the arteries stay open wider, and the same amount of arousal gets you further.
Two things follow from that. It cannot work if you are not aroused, because there is no messenger to protect. And it does not make you want sex — that comes from the brain, not the blood vessels.

Side effects
